Patient Responsibilities
Effective health care requires that patients participate in their care. Patients are responsible for providing information about past illnesses, hospitalizations, medications, and other matters related to their health. To participate effectively in decision making, patients must take responsibility for asking questions when they do not fully understand information and instructions.
Patients are responsible for informing their physicians if they anticipate problems in following the prescribed treatment. Patients are responsible for providing necessary information for insurance claims and for working with our staff to make payment arrangements, when necessary. We accept most major insurances, and co-pays are expected at the time of your visit. Please make sure you bring your insurance and referral information with you to your appointment.
